Job Description
Summary
This position provides support to patients and their families by offering educational resources about their diagnoses, helping them understand their condition and available treatment options. Guide them through the often complex healthcare journey by assisting with diagnostic testing, coordinating care, explaining the treatment process, and connecting them with appropriate resources to ensure informed decision-making and continuity of care.
This position is located at our McNair Campus.
Job Duties
- Serves as the primary liaison between patients, providers, nursing staff, and ancillary departments.
- Coordinates patient care across the continuum, including referrals, diagnostic testing, procedures, specialty consultations, and follow-up appointments.
- Conducts proactive outreach to patients regarding upcoming appointments, test results, and care plan adherence.
- Identify and address barriers to care, including transportation, financial concerns, scheduling conflicts, insurance issues, and health literacy.
- Educates patients and families on diagnoses, treatment plans, medications, procedures, and available resources.
- Monitors patient progress and ensure timely completion of recommended testing and follow-up care.
- Facilitates communication between primary care providers, cardiologists, surgeons, and other specialists to promote coordinated care.
- Tracks referrals, prior authorizations, and diagnostic orders to ensure timely completion.
- Reviews patient work queues and reports to identify overdue appointments, pending tests, or gaps in care.
- Documents patient interactions, interventions, and care coordination activities in the electronic medical record (EMR).
- Collaborates with physicians, advanced practice providers, nurses, and administrative staff to improve clinic workflow and patient access.
- Assists with quality improvement initiatives, regulatory compliance, and performance metrics related to cardiovascular care.
- Supports population health initiatives by identifying high-risk patients requiring additional care coordination.
- Monitors and reports key performance indicators such as appointment completion rates, referral turnaround times, patient satisfaction, and care coordination outcomes.
- Maintains knowledge of cardiovascular disease processes, organizational policies, and evidence-based patient navigation practices.
- Perform other duties as assigned to support departmental operations and patient care.
Minimum Qualifications
- High School diploma or GED.
- Four years of relevant experience.
Preferred Qualifications
- Experience in a Cardiology setting.
- EPIC experience.
