To demonstrate support of the hospital mission and vision by providing nursing, case management/disease management services to clients and families to help in the coordination and management of individuals’ health needs through the full continuum, as needed. The Centers Registered Nurse demonstrates the ability to assess, coordinate, plan, monitor, implement and evaluate the patient’s plan of care while ensuring and facilitating the achievement of optimal quality, clinical and financial outcomes. Performs assessment of clinical and life issues for the patients of COH. Provides professional nursing services in support of the physician, in the direct and indirect care of patients.

The successful candidate:

  • Must be able to work FT Monday-Friday, 8a-4:30p.

  • Provides disease management with an understanding of oncology, chemotherapy and the side effects, is able to monitor and address symptoms. Works with patients and families to provide an understanding of their disease, their treatment plans, and address patient quality of life issues. Documents assessment and interventions entered in a timely manner in the electronic medical record. Collaborates with, and supports provider team with daily assistance in managing and coordinating patient care. Evaluates and assesses health issues presented by patients and their caregivers and communicates pertinent and necessary issues to the physician.

  • Effectively and efficiently triages patient medical issues either in person or by telephone. Intervenes appropriately with use of orders and/or consultation with the interdisciplinary health care team. Provides professional nursing services to address patient’s health and other related issues in cooperation with other clinical and clerical team members.

  • Meets patients at follow-up appointment and supports the physician to ensure patient has all information to make informed decisions regarding treatment and how to access all ancillary services appropriately. Documents timely and thoroughly in the electronic medical record.

  • Works as a part of the team as a resource to facilitate communication and services between physicians, patients, families and other healthcare team members including external health care agencies, to facilitate the plan of care, including home services and medical equipment if needed. Provides patient education, utilizing appropriate resources to meet the needs of the patient and documents education in SCM.

  • Performs tasks as outlined in facility policies and procedures, as required by regulating agencies and according to best nursing practices, including but not limited to central line care, patient education, procedure assist and medication administration.

  • Works as a part of the health care team to provide excellent customer service to increase patient satisfaction and retention. Works to achieve positive outcomes and resolution of variances. Covers for other members of the team to ensure efficient delivery of service to the patient. Receives and returns patient calls, and provides care for drop-ins and appointments, in a timely manner in accordance with hospital “turn-around time” goals.

Qualifications

Your qualifications should include:

  • Current Georgia RN license from an accredited college

  • BSN required or enrolled within 6 months of hire and BSN completed within three years from hire date

  • 1 year clinical oncology experience preferred

  • BLS certification required. ACLS preferred

  • OCN Certification preferred

  • Bio/Chemo Certification preferred

Salary / Pay Rate Information:
Pay Rate: $32.85 - $54.13 / hour

The estimated pay scale represents the typical [salary/hourly] range City of Hope reasonably expects to pay for this position, with offers determined based on several factors which may include, but not be limited to, the candidate’s experience, expertise, skills, education, job scope, training, internal equity, geography/market, etc. This pay scale is subject to change from time to time.
