Job Description
Summary
The department of Ambulatory Operations has an opening for a Health Coach who will mentor and coach patients to help them achieve their personal goals. This position is located in the Fannin Tower and is an onsite role.
Job Duties
- Provides coaching sessions to patients around their health goals.
- Co-creates action plans and goals to meet the patient’s needs.
- Monitors the patients progress in goal completion.
- Maintains accurate documentation of patients in health systems.
- Conducts outreach calls and follows up on patient engagement.
- Facilitates health coaching programs.
- Other duties as assigned.
Minimum Qualifications
Required: Bachelor's degree.
Required: Two years of relevant experience.
