Job Description

Conducts a detailed psychosocial assessment to determine the candidacy for solid organ transplant or living donor. Participates on an organ specific Selection Committee; presents the mental health, social status, and functional abilities of candidates. Must have familiarity with alcohol and substance abuse disorders and make recommendations regarding rehabilitation or counseling prior to listing. Offers crisis interventions to patients and families with psychosocial needs. Assists with the transition of care for high risk patients.

Responsibilities:

  • Completes psychosocial assessment for selection of a candidate for organ transplant and living donor.
  • Conducts psychosocial assessment and interventions; makes treatment recommendations and engages patients/families in adherence to the treatment plan by reducing or eliminating, to the extent possible, the social, psychological, financial, behavioral and regulatory barriers to successful medical outcomes.
  • Deals with families exhibiting complex family dynamics.
  • Communicates with interdisciplinary team regarding the discharge planning status of all patients referred by them.
  • Educates patient/family and physician regarding post-acute options and addresses freedom of choice and financial concerns.
  • Coordinates transition of care planning.
  • As a member of a multidisciplinary team, participates in discharge planning for assigned patients in order to ensure a timely discharge and to provide appropriate linkage with post-discharge care providers and resources.
  • Serves as a resource person and provides counseling and intervention related to treatment decisions and end-of-life issues. Advocates for patient and family.
  • Provides intervention in cases involving child abuse/neglect, domestic violence, elderly abuse, institutional abuse and sexual assault.
  • Advocates for patient and family empowerment and independence to make autonomous health care decisions and access needed serves within the health care system.
  • Meets Core Social work competencies, including adhering to regulations.
  • Stays current with regulations which impact the care of patients and the Clinic's regulatory compliance.
  • Promotes individual professional growth and development and meets mandatory/continuing education, skills competency, supports department-based goals which contribute to the success of the organization; serves as a preceptor, mentor, and resource to less experienced staff.
  • Represents the department on committees.
  • Other duties as assigned.

Education:

  • Master's Degree in Social Work or Masters of Science in Social Administration (MSSA).

Certifications:

  • Current state licensure as a Social Worker LISW or LISW-S.

Complexity of Work:

  • The candidate must be advanced and able to practice independently in the state of Ohio per SW licensure law.
  • Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.
  • Must be able to work in a stressful environment and take appropriate action.

Work Experience:

  • Hospital Social Work experience preferred.
  • Exposure and experience in pre-acute and post-acute care.
  • Knowledge of community resources preferred.
  • Excellent communication and negotiation skills necessary to work collaboratively with multidisciplinary teams for optimal outcomes.
  • Demonstrated ability to work independently as well as with teams.
  • Must be flexible, open-minded and adaptable to change.
