Job Description
JOB SUMMARY:Performs assessment, treatment planning, intervention, monitoring, evaluation, and documentation on identified patient populations. Assesses and develops a care plan in collaboration with the heath care team. Effectively manages patients medical, environmental, and psychosocial needs to ensure their needs are being met.
Principal Duties:
-Participates with and engages patients in the assessment, planning, monitoring and evaluation of their care needs.
-With the healthcare team, develops an outcome-based plan of care, based on the patient's input and assessed needs. Implements and evaluates the plan of care as needed.
-Documents patient assessments, care plans and other relevant information in the electronic health record (EHR).
-Functions as a liaison between members of the healthcare team, the patient and, as appropriate, the family.
-Obtains and coordinates existing resources to meet patient needs.
-Helps facilitate transition to higher levels of care, including inpatient, residential and outpatient treatment programs, as clinically indicated.
-Serve as member of interdisciplinary teams consisting of medical providers, mental health providers, psychiatry providers, and nutritionists to manage the complex needs of patients.
-Demonstrate a commitment to multiculturalism and serving underrepresented/underserved populations.
-Participates in the education of peers and learners.
